New Age Athlete Participation Waiver
I, {name}, hereby acknowledge that I am voluntarily participating or allowing my child to participate in activities at New Age Athlete, located at 500 W 32nd St Charlotte, North Carolina. I understand that these activities may involve certain risks, including but not limited to, the risk of injury due to physical exertion, equipment malfunction, and the actions of other participants.
In consideration of being allowed to participate in these activities, I agree to assume all risks associated with my participation. I understand and acknowledge that New Age Athlete, its owners, employees, and affiliates shall not be liable for any injuries, damages, or death/losses that I may sustain as a result of my participation.
I hereby release, waive, discharge, and covenant not to sue New Age Athlete for any and all liability to me, my personal representatives, heirs, and next of kin for any and all claims, demands, losses, or damages on account of injury, including but not limited to, death or damage to property, arising out of or in connection with my participation in the activities at New Age Athlete.
I understand that it is my responsibility to consult with a physician prior to participating in any physical activity at the New Age Athlete and to ensure that I am physically fit and able to participate safely. I agree to abide by all rules and regulations of New Age Athlete and to follow the instructions of the New Age Athlete's staff and trainers.
I hereby grant permission to New Age Athlete to use photographs, video recordings, or any other media taken of me during my participation in activities at New Age Athlete for promotional or marketing purposes.
I have read this waiver and release of liability, fully understand its terms, and voluntarily agree to its provisions. I acknowledge that I am giving up substantial legal rights, including the right to sue New Age Athlete.
